Who are the Children and Young People Services and Send Teams and what do we do?
The Children and Young People Services/SEND teams are working to ensure that all children and young people get the right support at the right time so that they can live a happy and fulfilled life.
We work alongside local authorities and health providers, as well as other stakeholders including parent/carer groups, children and young people and their families and schools to make sure that health services for children and young people in hospitals, GP surgeries and the community consider the needs of the whole person.
We want to improve the health and wellbeing of children and young people from 0-25 years of age and to reduce any inequalities in healh through:
- Effective early identification of needs with appropriate assessments
- Services that meets the needs of our young people
- Smooth transition across Education, Health and Care at every stage of a young person's life, through to adult life
- Communication and engagement between young people, families, and their services
- Skilled leadership and staff capabilities to delive the services
